Grade 1 and 2 muscle strains (partial tears) are typically treated without surgery. Grade 3 injuries (complete muscle or tendon ruptures) are a different matter. A complete Achilles tendon rupture, a full quadriceps tear, or a distal biceps rupture will not heal correctly without surgical repair. A significant proportion of fibres are torn but the muscle is structurally intact. Noticeable weakness, bruising, and swelling. Treatment: physiotherapy, possible PRP injection, and 3–6 weeks structured rehabilitation.
The entire muscle or tendon is completely torn through. A palpable gap may be felt. Total loss of that muscle's function. Surgical repair is usually required for major muscle groups. Early intervention gives the best outcomes.
